Service Description
Facility Fees $65 – Fresno County Medical Facilities $100 – Madera & Kern County Medical Facilities The facility fee covers the additional time, coordination, and specialized requirements associated with hospital, rehabilitation center, nursing home, assisted living, and long-term care facility appointments. Additional Fees $15 per notarized signature Travel fees apply based on location Additional charges may apply for after-hours appointments, document delivery, courier services, excessive wait times, or other special requests
Cancellation Policy
Upon booking an appointment, a non-refundable deposit and/or full payment equivalent to the quoted service amount may be required to confirm the appointment. Generally, the deposit collected will be equivalent to the mobile travel fee and will be applied toward the final balance due at the completion of services. Complimentary telephonic consultations are excluded. Deposit fees and payments will be collected electronically at the time of scheduling.**Please note that all deposits are non-refundable regardless of cancellation, rescheduling, changes to the appointment, or failure to appear. Same-day cancellations and missed appointments may result in forfeiture of all payments made.*Additional fees may apply depending on travel distance, printing requirements, scanbacks, wait time, after-hours appointments, hospital or jail visits, and special accommodations requested.
